Wij zijn weer open voor inschrijvingen voor patiënten die in Wageningen wonen, wij vragen u het inschrijfformulier in zijn geheel in te vullen. Na dat u het inschrijfformulier bij ons heeft ingevuld mag u contact op nemen met uw huidige huisarts voor overdracht van het dossier. Pas als wij het dossier van uw vorige huisarts hebben ontvangen is de inschrijving compleet.

We are open for registrations again for people that live in Wageningen. We ask you to complete the registration form in its entirety. After you have completed the registration form with us, you may contact your current GP to transfer the file. The registration will only be complete once we have received the file from your previous GP. If you are new in the Netherlands and you don’t have a previous Dutch GP you can fill that in.
